While A Certain Scientific Accelerator is essentially a spin-off of the original Magical Index series, in theory, you can watch it before the main anime. However, it is not recommended to do so. The characters appearing in the spin-off will not seem as interesting as they should if you decide to skip.
Will I miss out anything crucial to index storyline if I just skip railgun and watch the whole index series by itself ? Yes you can. But watching both will enhance the experience of both series.

The spinoffs aren't necessary to understand the main Index series, but personally, I enjoyed the Railgun spinoff a lot more than Index itself, so you could be possibly missing out on a lot of great stuff if you skip the spinoffs! But if you only want to watch Index, that should work, too.

The Sister's Arc is 4 episodes in Index and 14 in Railgun S as such the Railgun S version's enjoyment should clearly take priority, especially considering the fact that it is the primary arc of the entire Railgun series (and arguably the best arc of the entire show).
The original and spin-off series were produced by the same company. (The same team as well I believe) and never aired at the same time.

A Certain Magical Index III began airing in Japan on AT-X, Tokyo MX, and BS11 on October 5, 2018, and on MBS on October 6. Muse Communication began airing the series in Taiwan on i-Fun Anime Channel on October 7, 2018.
She then orders the forces to save her. The Knight Leader and Acqua join Kaori in defeating Carissa, but they struggle with her Curtana Original. Elizard arrives with the Union Jack that would give the British citizens to fight back. Carissa is finally defeated and gets punched away by Tōma. ITEM encounters soldiers led by their liaison, who wanted to retrieve Rikō due to her potential of becoming the #8 Level 5 esper, but they are saved by the villagers. Shiage and Accelerator negotiate for their safe return to Academy City. Elsewhere, Fiamma lands on the ground but gets heavily injured by Aleister for learning about Imagine Breaker. He is later found by Ollerus and Silvia. At a press conference, Krans announces the end of hostility between Russia and Academy City. Meanwhile, Leivinia and her assistant find an unconscious Tōma floating.
