The Great Hinamizawa Disaster is a recurring event in When They Cry and is completely dependent on the way that Rika inevitably dies in the story. In order for this event to occur, Rika must die at the hands of Takano Miyo. When it does occur, the disaster wipes out all of Hinamizawa and its residents.

In most scenarios, the main characters have no idea who is behind the murders, however the entire village or someone in the village is always suspected to be behind it, especially the Sonozaki family. The motive behind the killings is also never entirely clear, except revenge against enemies of Hinamizawa.
Keiichi is killed as many times as Mion, Irie Kyōsuke and Tomitake.
Despite only being killed five times on-screen, dialogue eventually confirms that she has been killed once for every arc (excluding Matsuribayashi-hen and Miotsukushi-hen) and all other unseen worlds. Rika is a younger classmate in Keiichi's school, and is in the same grade level as Satoko.
What is the true ending of this anime and manga, does everyone survive in the true ending? Well, the original ending is Matsuribayashi-hen in which, yes, everyone survives. But if you wanted to, you could think of Miotsukushi-hen as the true ending instead. Or the alternate version of Miotsukushi-hen.
Takano Miyo (鷹野 三四), born Tanashi Miyoko (田無 美代子), is the main villainess/antagonist in the original Higurashi no Naku Koro ni and Higurashi no Naku Koro ni Kai.
Rena suddenly stabs Keiichi with Satoko's knife to "save" him from Oyashiro-sama's curse. Keiichi starts to apologize, but Rena leaves Keiichi in the burning house. Earlier that same day, Rena goes to "save" Rika from Oyashiro-sama's curse, which is believed by Rena to be the cause of the whole tragedy.
Hinamizawa is based on the real world village of Shirakawa-go (白川郷 Shirakawa-go).

The Hinamizawa Syndrome (雛見沢症候群, Hinamizawa Shōkōgun) is a fictional organic brain disease of uncertain etiology, endemic to the residents of Hinamizawa. It appears to be caused by a type of parasite which can only be recovered from living tissue.
Miyo gives Satoko an inkblot test and tells her that her condition is improving. She also reveals that she plans on quitting her job at the clinic. When Satoko asks why Miyo seems to allude to her inevitable betrayal and acknowledges that she's being taken advantage of.
The episode and season ends with Satoko resolving to trap Rika in horrifying loops until she agrees to stay in Hinamizawa – no matter how long it takes. Unfortunately the curse of being a first time watcher struck hard during this episode.
Satoshi seems to be the only protagonist who survives in all arcs, but this may be because he rarely appears in any of them. His fate after the death of Irie is very ambiguous due to Takano Miyo's eagerness to vivisect victims of Hinamizawa syndrome.
