While the anime doesn't end on a cliffhanger — Misaki finally admits her feelings for Usui, and the pair of them get together as a couple — the manga actually takes the story quite a bit further...
In the manga, Tora and Misaki's relationship becomes a lot closer because of his involvement with Usui's transfer to Miyabigaoka and trip to England. Tora is the one who prepares her and flies with her to England to meet Usui.
As of Chapter 81 of the manga, Takumi gifted Misaki an engagement ring for her 18th birthday, all the while cryptically asking her to marry him, which she tearfully accepts. In the final chapter, she married Usui and adopted his surname "碓氷".
Afterwards Misaki And Takumi both go to watch the fireworks, wearing Romeo and Juliet costumes, and admit their feelings for eachother, and kiss for the first time. Although this is the end of the anime, the manga keeps on going till chapter 87.
She usually addresses Misaki fondly as "sis" or "Onee-chan" and appears to have a relatively good relationship with her. The two sisters also look very much alike, except that Suzuna has shorter hair than Misaki which she wears in two pigtails most of the time.
In chapter 32, Usui confesses to Misaki during the school's firework festival and they share a kiss, though the relationship does not become formalized until Misaki herself confesses her love to him in chapter 57.
With Kuuga's words on her mind, Misaki confesses to Usui that she wants to be with him even though she does not understand her feelings. They share a kiss during the display of fireworks. After he confesses his love for her, they leave the classroom while holding hands.
The Confession is the 57th chapter of the Kaichou wa Maid-Sama!

While Misaki and Usui don't get married in the anime series, they do eventually get married in the manga. That happens after they get engaged in Chapter 81. Chapter 85 takes place 10 years later and, by that point, Misaki is a diplomat and Usui is a doctor and, yes, they are married to each other.
The girls decide to help out at the resort by hosting a one-day Maid café event to attract customers. Misaki, though, is embarrassed to come out to help wearing a sexy swimsuit. Usui gives her a hickey on her back, forcing her to wear a t-shirt to hide it.
Finally, in chapter 85, 10 years after Takumi and Misaki graduate, they get married. At this point, Takumi is a famous doctor in England working for the walker family, and Misaki is a world -enowned diplomat. After their marriage, they escape on a honeymoon back home, and meet up with their friends.
The anime started streaming on Netflix on July 29, 2020. So, it took almost 10 years for the anime to reach the big platform. However, in the month of October 2021, along with many other famous animes like K-on, Love, and so on, the series was removed from Netflix. Fans can still watch the anime on Hulu.

The series adapted Hiro Fujiwara's LaLa magazine manga of the same name, which began its run back in 2005. There was more to the manga than what the anime got around to adapting, but despite this continued material, there's been no second season.
