Short answer: Kaneki ate part of Hide to stay alive. Hide encouraged him to do so. He meets up with kaneki at the coffee shop and they have a heart to heart. The ending shows kaneki carrying a supposedly dead Hide through the CCG ranks and sets him down to fight Arima.

But the manga implies that Kaneki dies after the Anteiku incident at the hands of Arima. Note that the anime also implies some sort of confront between the two not necessarily a fight, at the end of season two.

Kaneki assumed he'd devoured his friend but Hide later turned out to be alive and living under the alias Scarecrow in Tokyo Ghoul:re. The friends eventually reunited and Hide revealed Kaneki had eaten part of his face but he'd survived the ordeal.

It's ingrained into his muscle memory. The first time Kaneki is shown doing this comes from Chapter one of the Tokyo Ghoul manga, when Kaneki scratches his chin after being confronted by Rize for staring at her too long.
a scene was shown in which Kaneki and Hide were talking at the top of a building. Later on, Kaneki asks Hide to show what he (Kaneki) did to him (Hide). This implies that Kaneki did attack/mutilate Hide but could not remember it himself. Also, since Hide did not harbor feelings of anger or regret against Kaneki throughout the manga, it can be said that he either let Kaneki eat a part of him or he might have tried to fight back and failed but forgave Kaneki for what he did. Personally, I think the former was what happened, given how Nagachika is.
Hide started bleeding shortly after Kaneki woke up, however, Kaneki had passed out from blood loss and was having difficulty healing. #N#Then there's the weird dream Kaneki had that makes me believe that maybe he was, like, sleep-feeding or something.
During the skip between his blackout and his awaking Hide told Kaneki everything he needs to know. Most likely what Hide is going to do from now on as well, since Ken seemed very calm even after Hide was gone, which can only mean he knows Hide is safe.

I wouldn’t be surprised if Hide actually brought him human flesh, we are talking about the boy who stole a ghoul corpse’s clothes. Another possibility is Kaneki ate his own flesh, because we only get the statement “the sweet taste of blood lingered in my mouth“, which in no way shows he fed on human flesh.
Kaneki has gone out of his mind, babbling irrationally in a state of being half kakuja. Hide lets Kaneki eat his face to get back his strength.
