The Quincy refused to accept the situation. As time went on, the number of Quincy increased and the cycle of souls became more unstable, forcing the Shinigami to reluctantly wipe out the Quincy in order to maintain the balance of the world.

Although Ichigo has an ability to endure combat situations that nearly no other soul reaper could, mainly because of the hollowfied Ichigo living within him. That being said it seems that Ichigo may have more than one life but he is not immortal yet.
However, he had zero control over it, so I don't think you could really call it Ichigo's. That wasn't a Vasto Lorde though. It was an artificial tracking Hollow called "White". The reason it was so strong is because it was made from Shinigami souls rather than human souls, which are much stronger on average.
As he pulls his talons out of Ichigo, who collapses, Grand Fisher criticizes him for sending away his allies and throwing himself at the Hollow in a fit of blind rage, which resulted in him playing right into Grand Fisher's hands, and declares that Ichigo will die without even having wounded him.

Meanwhile, Yammy has his arm reattached by an arrancar surgeon, and Ulquiorra explains that Grimmjow lost his rank as an Espada when he lost his arm. Yammy is revealed to be the Tenth Espada.
